What are facial exosomes?
Exosomes are nanometric extracellular vesicles (30 to 150 nanometres in diameter) produced naturally by cells such as stem cells. Unlike cells themselves, exosomes have no nucleus and no capacity for division: they are intercellular messengers that transport functional proteins, mRNA, microRNA and regeneration signals between cells.
In the context of aesthetic and regenerative medicine, stem cell-derived exosomes are considered an advanced therapeutic line within the regenerative medicine field. Their mechanism of action is specific: once incorporated by dermal fibroblasts, they help activate the genes responsible for producing type I collagen, elastin and hyaluronic acid, while also supporting the modulation of inflammatory mechanisms associated with skin aging.
A single millilitre of exosome preparation can contain a very large number of vesicles loaded with regenerative signals. This represents a distinct and complementary way of delivering biological information to skin cells — different from conventional PRP from the patient's own blood.
How do exosomes work in the skin?
To understand why exosomes have been incorporated into facial regenerative medicine, it helps to break their mechanism into three steps that occur in sequence in the days and weeks following application:
1. Cellular signalling. Once the preparation reaches the dermis, skin cells — mainly fibroblasts — absorb the exosomes and read the information they carry (proteins, mRNA and microRNA). This is essentially an exchange of messages: the vesicles instruct the cell to activate certain repair programmes that slow down with aging.
2. Biostimulation and extracellular matrix. That signal promotes the synthesis of type I and III collagen, elastin and endogenous hyaluronic acid, and helps modulate the activity of matrix metalloproteinases (MMPs) — the enzymes that break down the skin's support matrix. The expected result is skin with improved firmness, thickness and elasticity.
3. Inflammation modulation. Exosomes also have an anti-inflammatory effect, which helps calm reactive skin and supports recovery after procedures such as chemical peels or laser treatments. This is why one of their most valued applications is post-procedure support.
It is important to be honest about expectations: exosomes do not fill or lift structures the way hyaluronic acid or thread lifts do. They work at the level of skin quality — texture, radiance, firmness — and their effect is progressive. The extent and duration of the result vary from patient to patient and are assessed during the consultation.
Molecular components of the Skinretin Xpert protocol
mesoestetic is a well-known Spanish medical dermo-cosmetic company, recognized internationally for products such as the cosmeceutical cosmelan® and mesotherapy protocols. With more than 35 years in the market, its technology is used by dermatologists and plastic surgeons worldwide.
The Skinretin Xpert Pack is mesoestetic's exosome protocol. It combines stem cell-derived exosomes with a clinically proven synergy of regenerative actives: microencapsulated retinol, stabilized vitamin C and high- and low-molecular-weight hyaluronic acid. This combination addresses skin regeneration through multiple simultaneous pathways:
-
1
Stem cell-derived exosomes Activate fibroblasts, stimulate the synthesis of type I and III collagen, elastin and endogenous hyaluronic acid. Reduce the expression of matrix metalloproteinases (MMPs) that degrade the extracellular matrix.
-
2
Microencapsulated retinol Stimulates cellular renewal and epidermal turnover. Improves texture, reduces fine lines and evens skin tone. Microencapsulation promotes controlled release to minimize the risk of irritation.
-
3
Stabilized vitamin C + hyaluronic acid Vitamin C boosts collagen synthesis and acts as a potent antioxidant. Dual-molecular-weight hyaluronic acid hydrates at depth and on the surface, creating an immediately visible plumping effect.
The following table summarizes the role of each component in the protocol. No single active works alone: the value of the pack lies in its synergy. The intensity of each step is adjusted to your skin type during the evaluation consultation.
| Component | Primary action | Expected skin benefit |
|---|---|---|
| Stem cell exosomes Core | Cellular signalling and biostimulation | Firmness, regeneration and inflammation modulation |
| Microencapsulated retinol | Cellular renewal and epidermal turnover | More even texture and attenuated fine lines |
| Stabilized vitamin C | Antioxidant + collagen synthesis | Radiance and protection against oxidative damage |
| Hyaluronic acid (dual weight) | Deep and surface hydration | Hydrated skin with an immediate plumping effect |
There is no single "best component" in isolation: the value of the protocol lies in the synergy. Dr. Jenny defines the combination and application technique based on your skin's anatomy and condition during the consultation.
Benefits of exosome treatment
Who is a candidate?
The Skinretin Xpert exosome treatment is especially indicated for:
- Patients between 35 and 60 with signs of skin aging who seek regeneration without surgery.
- Patients in post-procedure recovery (deep chemical peel, fractional laser, facial surgery) who want to accelerate healing.
- Patients with sun-damaged skin or pigmentation who seek genuine rejuvenation results.
- Sensitive or reactive skin that cannot tolerate strong acids or aggressive treatments.
- Patients wanting to enhance and maintain the results of facial surgery (facelift, blepharoplasty, rhinoplasty).
- Those seeking advanced regenerative technology in non-surgical aesthetic medicine.
When exosomes are not the best option
On medical grounds, it is important to be clear about limitations as well. Exosomes do not replace surgery: when there is significant laxity, excess skin or advanced facial descent, surgical lifting delivers a clearly superior result, and exosomes work better as a complement than a substitute. They are also not the indicated treatment if what you are looking for is volume or structural repositioning: for that, hyaluronic acid and thread lifts are more appropriate.
Additionally, the treatment is not indicated during pregnancy and breastfeeding, in the presence of active infections or lesions in the treatment area, in autoimmune diseases or active oncological processes without authorization from the treating physician, or if there is a known allergy to any component of the preparation. The prior evaluation consultation is precisely the moment to review your medical history and honestly decide whether exosomes are the best option for your case.
The procedure, step by step
An exosome session with Skinretin Xpert takes approximately 60 minutes in the clinic. The application can be performed with microneedles (microneedling), direct mesotherapy or enhanced transdermal absorption techniques, depending on the protocol indicated for each patient.
-
1
Skin cleansing and preparation Double enzymatic cleanse to remove all traces of sebum, makeup and contaminants. Preparation is key to optimizing the absorption of the preparation. Topical anesthesia is applied if microneedling is planned.
-
2
Application of the exosome preparation The Skinretin Xpert preparation is applied to the skin via superficial mesotherapy or microneedling, depending on the indicated protocol. This allows the exosomes to reach the dermal layers where fibroblasts act. The specific technique is defined during the evaluation consultation.

Post-treatment serum and sun protection A calming serum and high-SPF sunscreen are applied. The skin may show mild redness that resolves within 24 hours. Strict sun protection is recommended for 5–7 days.

One of the advantages of exosomes over more aggressive treatments is their minimal recovery: most patients resume their daily routine on the same day. Even so, the first few days are the most important for the final result, while the skin integrates the actives and regeneration begins.
What to expect during the first few days
It is normal to experience mild redness, a sensation of warmth or minimal flaking, especially when the application is performed with microneedling. These reactions typically resolve within 24 to 72 hours. The skin may feel somewhat more sensitive to touch and reactive to sunlight during the first week, which is why sun protection is essential.
Recovery recommendations
- Apply high-factor sun protection strictly for 5–7 days and reapply throughout the day.
- Avoid direct sun exposure, tanning beds, sauna and swimming pools during the first few days.
- Do not use strong acids, home-use retinoids or exfoliants during the first 48–72 hours.
- Keep the skin clean and hydrated, and postpone makeup for the first few hours if the application was performed with microneedling.
- Respect the intervals between sessions as indicated — the result builds progressively across the full cycle.
Although reactions are usually mild and transient, if you notice growing redness, intense pain, signs of infection (heat, discharge) or any unexpected reaction, contact the clinic. Cycle follow-up is included and allows us to accompany your progress and address any concerns.
Exosomes vs. other rejuvenation options
Exosomes do not compete with other treatments — they complement them. Each option addresses a different problem more effectively, and understanding what each one does helps set the right expectations. This honest comparison summarizes the role of each alternative:
| Option | Best suited for | Surgery | Downtime |
|---|---|---|---|
| Exosomes Skin quality | Texture, radiance, firmness and regeneration; post-procedure support | No | Minimal (24–72 h) |
| PRP (platelet-rich plasma) | Biostimulation with growth factors from patient's own blood; requires blood draw | No | Minimal |
| Hyaluronic acid | Volume loss and deep creases; does not improve skin quality | No | Minimal |
| Thread lifts | Mild to moderate laxity; repositions and tightens tissue | No | Minimal (2–5 days) |
| Surgical facelift | Severe laxity and excess skin; superior result in those cases | Yes | Extended |
In practice, exosomes are often integrated into a broader plan: they enhance the quality of the skin on which fillers, thread lifts or surgery will act. The evaluation consultation is precisely the moment to decide which combination makes most sense in each case, and in what order.
