What are PDO threads?

PDO (polydioxanone) threads are bioabsorbable sutures that have been used in cardiac and orthopedic surgery for decades. Their application in aesthetic medicine as a non-surgical lifting tool is based on two simultaneous mechanisms: mechanical tissue repositioning (immediate lift) and the biological response they trigger in the skin (progressive collagen neosynthesis).

When inserted under the skin through fine needles or cannulas, barbed or smooth PDO threads grip the soft tissue and reposition it superiorly, creating an immediate visible lift. At the same time, the foreign body response to the thread stimulates fibroblasts to produce new collagen around the thread — maintaining and improving the result even as the thread itself biodegrades over 6 to 9 months.

Dual mechanism

The thread lift works on two levels simultaneously: an immediate mechanical repositioning of descended tissue, and a biological collagen stimulation that progressively improves skin quality and firmness over the following months — long after the thread itself has dissolved.

Types of PDO threads

Barbed (cog) threads

These are the "lifting" threads. They have small bidirectional barbs along their length that hook into the subcutaneous fat and SMAS layer, providing the mechanical tissue repositioning. They are used for cheek lifting, jowl correction, neck tightening, and brow elevation.

Smooth mono threads

These are inserted in a mesh pattern to stimulate collagen production and improve skin quality without mechanical repositioning. They are ideal for fine lines, skin texture improvement, and as a complement to barbed threads to enhance overall skin quality.

Twisted/screw threads

These have a twisted or spiraled structure that creates more volume and stimulation than smooth mono threads. They are used for adding subtle volume and firmness in areas like the cheeks, under-eyes, and lips.

The procedure

The duration of the effect varies for each patient and is assessed at the consultation. The threads biodegrade gradually, while the collagen stimulation prolongs the result. Many patients schedule a periodic touch-up to maintain results. Results are individual and depend on skin quality, lifestyle, and the areas treated.
